- A personalized digital model of care which, due to a sustained engagement between patients and caregivers, personalizes the treatment plan optimizing the patient benefits.
- The digital enabler of the project is a disease management platform for remote patient monitoring: the patient interacts daily with an app constantly monitoring his/her health conditions and a clinical dashboard collects all the self-reported data making them easily accessible to the Clinical team.

In a first user acceptance trial this e-health-enabled model of care for patients with chronic HF proved to be feasible and user-friendly, with high levels of adherence and perceived benefits, improving the quality of life of patients and the ability to deliver remote healthcare by the clinical team.
No HF-related hospitalization occurred and a significant increase in the perceived quality of life indicators was observed. The project is now scaling in a multicentric study in 4 sites with more than 300 patients involved. Now in the follow up phase.
The Model of care will also be tested in 2 accounts in Lombardy to verify the interoperability of patients data with the Federated Data Network deployed by the University of Milan.
